Beginning last season all officials must register with the ORA via form O-F-05.

Each official must fill out a Registration Form and choose one of the following two methods of submittal:

1. Mail it and the appropriate fee directly to the office. Once both are received you are considered registered.
2. Fill out the form and submit it to your local association registrar. The local association will send the form and payment to ORA office. Once the office receives the appropriate form and fee the official will be considered registered.

Do not fill out the form on the official's behalf.

The association registrar will register officials in the ORA Membership Database (electronic) or on the Official Registration Form (manual). Officials have been included in the membership lists and ORA Membership Database to assist in this process.
