All potential and actual injuries resulting from participation in Ringette must be sent immediately (within two (2) business days) to the O.R.A. office using the Risk Management and Safety Incident/ Accident Form (M-F-13).

The Form is available online under Membership Services in the Appendices Section of the O.R.A. Manual.
