USE OF ACCIDENT REPORT FORMS
All potential and actual injuries resulting from participation in Ringette must be reported by the local Association. Your Association is to pass this information to the O.R.A. office by using the Risk Management and Safety Incident Form (M-F-13). From these reports the Provincial Membership Services Committee can compile data to more effectively monitor and control risks within our sport. This reporting also provides some protection for the athlete should complications arise from the incident in the future. We would have a record of the incident on file in the office. If an injury occurs which results in out of pocket costs not covered by the injured personÍs insurance provider, an Accident Claim Form (PDF, 24Kb) should be completed and submitted to the O.R.A. office. After the office confirms the injured party is a registered member the forms are forwarded to the insurance company. From this point on the individual(s) will be dealing directly with the insurance company. These forms are in the Operating Manual - Appendices and can also be downloaded from our Website. Please letÍs all do our part in keeping our sport and itÍs participants safe. |
